Modify Risk Score Values

The system provides default score values and a default risk scale. Use these values, or customize both. By default, answers to questions are assigned a default Risk Score Value on a scale of 1 to 5, with 1 indicating lowest risk and 5 indicating highest risk. You may modify the assigned value for any or all questions within a completed questionnaire using this default 1-5 scale, or you can modify the Risk Score Scale and reassign values based on your own scale. To do so, please take the steps outlined below.

Modify Risk Score Values

  1. Click Menu in the top left corner > Administration & Tools > Risk Scores

  2. In your completed questionnaire, click a question to expand the content and display the responses and their associated scores.

  3. Replace the default score with a custom score

  4. Click Save at the bottom of the questionnaire

Modify Risk Score Scale

  1. Click Menu in the top left corner > Administration & Tools > Risk Scores

  2. In the top right corner of the questionnaire, click the blue pencil edit icon next to Score Range

  3. Enter the minimum and maximum range values and click Save.

Note: Modifying the scale range does not affect current score values already assigned to responses. You must manually update these values after you modify a score range for the new range to be useful.
